Capio Vårdcentral Nynäshamn ligger centralt i Nynäshamn, endast 10 minuters promenad från pendeltågsstationen. Vi är en medelstor vårdcentral med knappt 13 000 listade patienter.

Hos oss finns bland annat läkar- och sjuksköterskemottagning, diabetes-, astma/KOL- och hypertonimottagning, mottagning för äldre, psykosocialt stöd, medicinsk fotsjukvård, primärvårdsrehab samt specialiserad fysioterapi. Vi samarbetar tätt med BVC, BMM och har även Capio Geriatrik samt Karolinska Närvårdslaboratoriet i samma byggnad.

Arbetsbeskrivning

Vi söker nu dig som vill arbeta som sjuksköterska inom vår hemsjukvård (HSV). I dagsläget har vi ca 140 inskrivna i hemsjukvården, vilket emellanåt skiftar. Arbetet innebär att ge kvalificerad omvårdnad i patientens hem, utföra medicinska bedömningar och insatser samt samverka med övriga professioner kring våra patienter. Du arbetar självständigt men har alltid stöd av ett kompetent team i gruppen.

Vår verksamhet präglas av ett aktivt förbättringsarbete, nära teamarbete och korta beslutsvägar. Hos oss får du möjlighet att bidra till utvecklingen av hemsjukvården och delta i arbetet med att vidareutveckla våra samverkansformer.

Hemsjukvårdsläkare är en del av teamet.

Kvalifikationer

  • Du är legitimerad sjuksköterska och erfarenhet av arbete inom primärvård och/eller hemsjukvård är meriterande.
  • Du trivs med att arbeta självständigt, men även fungera väl i grupp, är trygg i din yrkesroll och har ett genuint patientfokus.
  • Du är ansvarstagande, flexibel, har god organisationsförmåga och intresse för förbättringsarbete.
  • B-körkort krävs för arbete inom hemsjukvården.

Övrig information

  • Provanställning 6 månader med möjlighet till fast anställning efter de första 6 månaderna.
  • Vi är en rökfri arbetsplats.

  • Startdatum efter överenskommelse.
