Als Begeleider ben je een vertrouwd gezicht voor onze bewoner. Je biedt begeleiding en ondersteuning bij het dagelijks leven en helpt bewoners om zoveel mogelijk zelf de regie te voeren.

  • Je voert het zorgplan uit en zorgt dat afspraken en doelen worden vertaald naar de dagelijkse begeleiding.
  • Je biedt structuur, duidelijkheid en ondersteuning die past bij de bewoner.
  • Je stimuleert bewoners om zelf keuzes te maken, vaardigheden te ontwikkelen en zo zelfstandig mogelijk te functioneren.
  • Je signaleert veranderingen in de situatie van bewoner en bespreekt deze met de persoonlijk begeleider.
  • Je denkt mee over mogelijke aanpassingen in het zorgplan en levert input vanuit jouw dagelijkse contact met bewoners.
  • Je rapporteert ontwikkelingen en bijzonderheden in het bewoner registratiesysteem.
  • Je levert waar nodig input voor het multidisciplinair overleg en neemt hieraan deel.
  • Je draagt bij aan een veilig, prettig en stimulerend woon-, werk- en leefklimaat.
  • Je werkt samen met collega’s, de persoonlijk begeleider en andere betrokken professionals.

  • Je hebt een passende opleiding voor het werken in de gehandicaptenzorg.
  • Je bent geduldig, betrokken en kunt goed luisteren en observeren.
  • Je kunt goed aansluiten bij verschillende bewoner en hun begeleidingsbehoeften.
  • Je signaleert veranderingen en weet wanneer je actie moet ondernemen of een collega moet inschakelen.
  • Je neemt verantwoordelijkheid voor je eigen werk en kunt zelfstandig handelen.
  • Je werkt graag samen en draagt actief bij aan een goede sfeer en samenwerking binnen het team.
  • Je communiceert duidelijk met bewoner, collega’s en andere betrokkenen.
  • Je kunt rustig en professioneel handelen wanneer situaties ingewikkeld of onverwacht zijn.
  • Je stimuleert de eigen regie en zelfstandigheid van bewoner.
