Als Leefondersteuner binnen het VPT-team (Volledig Pakket Thuis) in Leidsche Rijn en De Meern West ben jij een vertrouwd gezicht voor cliënten. Je ondersteunt bij dagelijkse activiteiten, brengt structuur aan waar nodig en hebt oog voor wat iemand gelukkig maakt. Samen met een betrokken team van zorgprofessionals kijk je naar wat wel mogelijk is, zodat cliënten de regie over hun leven behouden. Zo maak jij iedere dag het verschil en help je mensen om op een prettige, veilige en zelfstandige manier thuis te blijven wonen.

Dit ga je doen als Leefondersteuner

Als Leefondersteuner binnen het VPT-team in Leidsche Rijn en De Meern West ben jij er voor cliënten die graag zelfstandig thuis willen blijven wonen. Je ondersteunt hen bij hun dagelijkse leven, welzijn en zelfredzaamheid, bijvoorbeeld door te helpen bij het wassen, douchen en de toiletgang. Daarnaast bied je begeleiding, breng je structuur aan in de dag en heb je oog voor wat iemand nodig heeft om zich prettig en veilig te voelen. Samen met collega’s, naasten en andere zorgprofessionals zorg je voor ondersteuning die aansluit bij de wensen en mogelijkheden van de cliënt.

Je komt terecht in een dynamische wijk waar je werkt in Leidsche Rijn en De Meern West, een gebied met een diverse groep cliënten en een sterke focus op zelfstandig thuis wonen. Door de combinatie van moderne woonwijken en een groeiende zorgvraag krijg je de kans om echt een band op te bouwen met cliënten in de buurt. Je bent veel onderweg in de wijk en werkt nauw samen met collega's, mantelzorgers en andere zorgprofessionals. Juist die verbinding met de wijk, de afwisseling in je werk en de ruimte om persoonlijke zorg te bieden maken het werken in dit team zo aantrekkelijk.

Wat je van ons mag verwachten

  • Salaris volgens cao VVT, FWG 20: € 2.488 - € 3.064 (o.b.v. 36 uur).
  • Direct een vast contract voor 28 uur per week.
  • Reiskostenvergoeding, 8% vakantietoeslag en 8,33% eindejaarsuitkering.
  • Mogelijkheid om gebruik te maken van het meerkeuzesysteem, zoals extra verlof of een fietsregeling.
  • Toegang tot o.a. HeyCoach, budgetcoach en DigiCoach voor jouw ontwikkeling.
