Dan kijken we uit naar jouw sollicitatie. Waar ga je aan de slag? Ons team outreachende verslavingszorg biedt intensieve zorg en bemoeizorg op meerdere levensgebieden. Het doel is om persoonsgerichte en herstelondersteunende zorg te leveren op verschillende domeinen en met wisselende intensiteit. In onze herstelbenadering staat centraal dat we cliënten ondersteunen om, ondanks mogelijke beperkingen, de regie op hun eigen leven weer te pakken. Dit gebeurt onder andere door gebruik te maken van hoop, empowerment, verbinding en zingeving. Wij werken volgens het FACT-model (Flexible Assertive Community Treatment), waarin flexibiliteit, shared caseload en beschikbaarheid de basiselementen zijn. Daarnaast is er methadonbehandeling voor cliënten met een opiaatafhankelijkheid. Het team waarin je gaat werken is multidisciplinair samengesteld. Iedereen heeft zijn of haar eigen kennis en ervaring. En zo vullen we elkaar aan. We delen informatie, reflecteren op ons handelen én zetten humor in! Je werkt binnen kantoortijden, dus van 8:30 tot 17:00 uur en niet in het weekend. Wat ga je doen? Als ambulant hulpverlener heb je, na een inwerkperiode, een eigen caseload en je bezoekt de cliënten in hun eigen leefsituatie. Zo behandel je de psychiatrische problematiek en heb je structurele aandacht voor de somatische situatie van de cliënt.
