Ben jij een ervaren begeleider met coachende kwaliteiten, die rust brengt in complexe situaties en het team naar een hoger niveau tilt? Heb jij minimaal een hbo of mbo-4 diploma en meerdere jarenwerkervaring als Begeleider? Kom dan onze teams versterken! Wat ga je doen? Je gaat werken bij team Woont Schiedam-Zuid, in de wijk De Gorzen, samen met ongeveer 15 collega's. Het grootste deel van je werk (zo'n 70%) bestaat uit ambulante begeleiding van cliënten die voornamelijk in de buurt van de locatie wonen. Je werkt onregelmatige diensten en draait af en toe een weekenddienst (ongeveer eens in de 5 à 6 weken). Als Senior Woonbegeleider ondersteun je je collega's in hun professionele ontwikkeling. Je coacht hen in methodisch en herstelgericht werken en stimuleert samenwerking met het netwerk van de cliënt en de mogelijkheden in de wijk. Daarnaast help je om begrippen zoals tijdelijkheid, eigen regie en autonomie concreet toe te passen in het dagelijks werk. Daarnaast begeleid je zelf ook cliënten, vooral als er sprake is van complexe situaties. En waar nodig ondersteun je bij ADL. Dankzij jouw ervaring en aanwezigheid breng je rust, overzicht en stabiliteit. Je zet je actief in om het netwerk rond de cliënt te versterken, zodat die zoveel mogelijk zelf de regie kan nemen en kan werken aan persoonlijke doelen en herstel. Je werkt nauw samen met behandelaren en het netwerk van de cliënt. Je signaleert ontwikkelingen, deelt kennis en stimuleert samenwerking in én buiten het team en je ontvangt leiding van de meewerkend teamleider.
