Votre mission :

En tant qu’assistant·e social·e, vous occupez une fonction centrale de lien entre le bénéficiaire, son entourage, l’équipe interdisciplinaire et les partenaires extérieurs.

Vous accompagnez le parcours du bénéficiaire depuis la demande d’admission et tout au long de sa prise en charge. Vous veillez à la qualité des échanges entre les différents intervenants ainsi qu’au suivi administratif et social des dossiers.

Selon le service dans lequel vous exercez, vous travaillez auprès d’enfants et de jeunes accueillis en service résidentiel ou d’adultes.

Vos responsabilités :

Dans le cadre de votre fonction, vous serez notamment amené·e à :

· Organiser et assurer le suivi des demandes de prise en charge, des préadmissions, des admissions et des fins de prise en charge ;

· Assurer le suivi administratif et social des dossiers des bénéficiaires ;

· Être une personne de référence et de liaison entre le bénéficiaire, sa famille ou son réseau, l’équipe et les partenaires extérieurs ;

· Assurer les contacts et la collaboration avec les services et organismes partenaires ou mandants : AVIQ, ARS, SAJ, SPJ, services sociaux, services de santé, administrations et autres acteurs du réseau ;

· Soutenir et accompagner les familles et le réseau du bénéficiaire à travers des rencontres, concertations et réunions de suivi ;

· Participer activement à l’élaboration, au suivi et à l’évaluation du projet individualisé de chaque bénéficiaire ;

· Identifier les besoins sociaux et administratifs et contribuer à la recherche de solutions, d’aides, de droits et de relais adaptés ;

· Participer aux réunions d’équipe, aux concertations et à la réflexion interdisciplinaire ;

· Développer et entretenir le réseau de partenaires externes ;

· Contribuer à la vie du service et à l’évolution des pratiques professionnelles.
