Je bent verantwoordelijk voor het uitvoeren van het ondersteuningsplan van de cliënten die bij Zuidwester wonen en/of werken. Je werkt op woonlocaties en/of op dagbestedingslocaties. Jouw werkzaamheden: - Je helpt bij het onderhouden van (sociale) contacten en je zorgt voor structuur en veiligheid in een huiselijke omgeving. - Je bent aanspreekpunt voor zorgpartners, ouders en familieleden van de cliënten. - Je zorgt voor een zinvolle besteding van de dag. Je doet dit natuurlijk niet alleen, maar samen met je collega's. De sfeer is ongedwongen, we helpen elkaar en zorgen ervoor dat iedereen zich betrokken en gehoord voelt. Een manier van werken waarbij we ons verbonden voelen en samenwerken aan het welzijn van iedereen bij Zuidwester. Waar ga je werken? Je gaat aan de slag op onze dagbestedingslocatie Schengestraat 39a/b, een structuurgroep in Goes. De dagbesteding bestaat uit maximaal 20 cliënten, verdeeld over twee groepen.
