* Sie erfassen im Außen- und Innendienst den individuellen Bedarf der Kund:innen, planen entsprechende Maßnahmen, klären die Kosten und organisieren Unterstützungsangebote für stationäre und mobile Leistungen.

* Sie arbeiten auf Augenhöhe in einem interdisziplinären Team mit der Möglichkeit auf Home-Office und vernetzen sich mit der Wiener Gesundheits- und Soziallandschaft.

* Sie bearbeiten Anliegen im Kund:innenkontext und leisten bereichsübergreifende Schnittstellenarbeit mit relevanten Bereichen sowie Kooperationspartner:innen.

* Sie leiten Sofortmaßnahmen und Kriseninterventionen im Bedarfsfall ein.

* Sie setzen gezielt Impulse und sind zentral an der Weiterentwicklung von Leistungen und internen Abläufen beteiligt.

Setzen Sie Ihre Fähigkeiten wirksam ein

* Diplom Gesundheits- und Krankenpflege sowie mehrjährige Berufserfahrung im mobilen oder stationären Pflegebereich bzw. im Case Management

* Freude an interdisziplinärer Zusammenarbeit und Fähigkeit Prioritätensetzung sowie vernetztes Denken im Bezug auf soziale Systeme

Gehalt: regelmäßiges Entgelt gemäß KV-Schema II: Einstufung nach dem Kollektivvertrag des Fonds Soziales Wien von € 3.713,75 bis 4.180,07 Monatsbrutto je nach anrechenbaren Vordienstzeiten (40 Wochenstunden, bezahlte Pause, 14 mal jährlich) sowie jährliche leistungsabhängige Prämie gem.
