Jij hebt een regisserende/coördinerende rol en indien nodig kan je vanuit je functie ook kortdurende interventies doen om de klant snel en effectief te ondersteunen. Je gaat samen met de klant (en het aanwezige netwerk) met de hulpvraag aan het werk. Je start met één (of meerdere) huisbezoeken (keukentafel gesprek) waar de hulpvraag, onder andere op basis van de ZRM (Zelf Redzaamheid Matrix), inzichtelijk wordt gemaakt. Door een integrale diagnose en daarop afgestemde interventies is de effectiviteit van de ondersteuning optimaal. Aan de hand van de analyse maak je samen met het huishouden een ondersteuningsplan, een plan waarin beschreven wordt wat het huishouden/gezin gaat ondernemen om de zelfredzaamheid te herstellen en welke ondersteuning vanuit de professional nodig is. Hierbij schakel je het netwerk van het huishouden actief in. Het organiseren van eigen oplossingen is geen separaat traject maar een integraal onderdeel van de werkwijze. Je hebt altijd oog voor veiligheid in het huishouden. Indien je een onveilige situatie signaleert, worden er concrete veiligheidsmaatregelen in het ondersteuningsplan opgenomen. Ieder huishouden met een ondersteuningsplan heeft één aanspreekpunt, dat ben jij! Je monitort samen met de klant de resultaten van het plan. Wanneer het nodig is spreek je organisaties of collega's aan op het nakomen van afspraken. In jouw caseload word je ondersteund door collega's met casuïstiekoverleg, werkbegeleiding en intervisie. Waar nodig schaal je tijdig op naar de teamleider. Je werkt nauw samen met collega's van andere afdelingen en met partners die in het gebied actief zijn zoals: scholen, de huisarts, de wijkverpleegkundige, casemanagers dementie, het steunpunt mantelzorg, het veiligheidshuis, Eigen Haard...
