Ervaren Consulent Jeugd Beilen Functie Voor de gemeente Midden-Drenthe zoeken wij een ervaren Consulent Jeugd voor 32 uur per week. Je behandelt complexe jeugdcasuïstiek en hebt daarbij veel aandacht voor veiligheid binnen gezinnen. Je voert zelfstandig regie op hulpverleningstrajecten, werkt nauw samen met gezinnen en ketenpartners en beoordeelt welke ondersteuning vanuit de Jeugdwet nodig is. Voor deze opdracht zoeken we nadrukkelijk iemand die al ervaring heeft als jeugdconsulent binnen een gemeentelijke organisatie. Wat ga je doen? - Beoordelen van hulpvragen en bepalen welke geïndiceerde jeugdhulp nodig is; - Coördineren van hulpverleningstrajecten en bewaken van de voortgang; - Monitoren en evalueren van ingezette hulp en waar mogelijk tijdig afschalen; - Voeren van regie bij complexe casuïstiek; - Signaleren en beoordelen van mogelijke onveiligheid binnen gezinnen; - Uitvoeren van risicotaxaties en opstellen en bewaken van veiligheidsplannen; - Direct handelen wanneer sprake is van acute onveiligheid; - Samenwerken met gezinnen, scholen, zorgprofessionals, jeugdzorginstanties en andere ketenpartners; - Toetsen van toewijzingsplannen aan de Jeugdwet, gemeentelijke regelgeving en professionele richtlijnen; - Opstellen en beoordelen van onder andere ondersteuningsplannen, beschikkingen en PGB-aanvragen; - Zorgdragen voor volledige en zorgvuldige dossiervorming; - Adviseren van collega's binnen het sociaal domein over jeugdhulp en veiligheid; - Deelnemen aan casuïstiekoverleggen, intervisie en multidisciplinaire overleggen; - Bijdragen aan inhoudelijke ontwikkelingen rondom jeugdhulp en complexe veiligheidscasuïstiek. Functie-eisen - Je beschikt over een aantoonbaar afgeronde opleiding op minimaal hbo-niveau; - Je beschikt over een geldige SKJ-registratie. Het registratienummer en de geldigheidsduur moeten uit het cv blijken; - Je hebt minimaal 2 jaar aantoonbare werkervaring als jeugdconsulent binnen een gemeentelijke org...
