Binnen de gemeente Middelburg zoeken wij een Wmo-consulent ter tijdelijke vervanging van een collega. Je komt terecht in een betrokken team dat zich dagelijks inzet voor inwoners die hulp nodig hebben, met name op het gebied van huishoudelijke ondersteuning en voorzieningen zoals rolstoelen, vervoer en woningaanpassingen. Wat ga je doen? Als Wmo-consulent ben je verantwoordelijk voor het volledige proces vanaf de melding tot en met de evaluatie van de geboden ondersteuning. Je werkt binnen de afdeling Toegang, samen met jeugdconsulenten, casusregisseurs en ondersteunende collega's. Je werkzaamheden bestaan uit: - Het voeren van gesprekken met inwoners (telefonisch en via huisbezoeken) om hun hulpvraag in kaart te brengen - Het analyseren van de situatie en bepalen van passende ondersteuning - Adviseren over oplossingen, voorzieningen en zorgmogelijkheden (voorliggend en maatwerk) - Signaleren van risicovolle situaties en hier adequaat op handelen - Het inzetten en monitoren van ondersteuning en voorzieningen - Het opstellen van rapportages en evalueren van trajecten Je richt je voornamelijk op huishoudelijke ondersteuning en voorzieningen, maar pakt ook aanvragen op binnen begeleiding en dagbesteding. dit is werken bij koen - een vast contract en alle voordelen van in loondienst werken - uitdagende opdrachten bij afwisselende opdrachtgevers - veel ontwikkelkansen zoals opleidingen, trainingen en kennisses...
