Neben bestmöglicher Pflege und medizinischer Versorgung legen wir besonderen Wert auf einen selbstbestimmten und abwechslungsreichen Alltag. ... 1 DGKP Diplomierte Gesundheits- und Krankenpflegeperson (AG/R) - all genders 1160 Wien, Pflegekrankenhaus Seeböckgasse, 37 Wochenstunden, Voll-/Teilzeit

Das sind deine Aufgaben: * Professionelle, ganzheitliche Pflege und Betreuung älterer, multimorbider Patient*innen im akutgeriatrischen Setting * Umsetzung evidenzbasierter Pflegeprozesse in Zusammenarbeit mit Advanced Practice Nurse * Interprofessionelle Zusammenarbeit mit Medizin, Therapie, Sozialarbeit, Entlassungsmanagement und anderen Berufsgruppen * Unterstützung der Patient*innen in der Remobilisation zur Erlangung der Alltagskompetenzen * Mitgestaltung funktionierender Übergänge zu extramuralen Angeboten, Rehabilitation und Langzeitpflege * Aktive Teilnahme an Qualitäts- und Verbesserungsprozessen * Dokumentation und Pflegeplanung nach aktuellen fachlichen Standards

Das bringst du mit: * Abgeschlossene und in Österreich anerkannte Ausbildung zur diplomierten Gesundheits- und Krankenpflegeperson (DGKP/BScN) * gültigen Eintrag im Gesundheitsberuferegister * Berufserfahrung in Akutpflege, Geriatrie, Rehabilitation oder verwandten Bereichen von Vorteil * Freude an der Arbeit mit älteren, multimorbiden Menschen und Interesse an der akutgeriatrischen Versorgung und Remobilisation * Bereitschaft, neue Strukturen mitzugestalten und innovative Versorgungsmodelle mitzutragen * Fachlich fundierte, klare und empathische Kommunikation * Strukturierte, lösungsorientierte Arbeitsweise
