Met deze doelgroep zijn je werkweken nooit hetzelfde. Jouw verantwoordelijkheden zijn dan ook erg divers. Allereerst ben je caseloadhouder van een aantal patiënten. Je caseload is dynamisch, waarin de behandeldoelen veelal gericht zijn op verslavingszorg en psychiatrie. Veel van onze cliënten zijn al langere tijd in zorg, maar ook hebben wij op sommige poli's veel inloop van nieuwe cliënten. Als caseloadhouder ben je eerste aanspreekpunt en verantwoordelijk voor de coördinatie van behandeling van patiënten. Dit betekent ook dat je actief zorg mijdende patiënten moet benaderen. Voor jouw caseload draag je actief bij aan behandelplannen en gebruikt deze als richtlijn voor de te leveren zorg. Ook bij andere patiënten lever je input. In deze functie ben je medebehandelaar in een multidisciplinair team, samen zijn we verantwoordelijk voor goede behandeling van al onze patiënten. Je verantwoordelijkheid reikt dus verder dan je eigen caseload. Sommige dagen zal je ook ingezet worden als omloopverpleegkundige. Alle patiënten met hulpvragen komen dan eerst bij jou. Vervolgens is het aan jou de taak om taken uit deze vragen te filteren, verder te verwijzen of direct actie te ondernemen. Verder werken we nauw samen met huisartsenpraktijk Dr. Valckenier van de GGD. Als omloopverpleegkundige voer je ook triage uit voor de patiënten die binnenlopen met somatische problemen en bepaal je de urgentie. Of je ondersteunt de huisarts tijdens het spreekuur waar nodig. Dit doe je zowel op de poli als soms in de opvanghuizen waar cliënten wonen. Naast deze werkzaamheden zit je sommige dagen achter onze balie, waa...
