Functieomschrijving - Zelfstandig ambulante ondersteuning bieden met methodieken zoals oplossingsgericht werken, systeemgericht werken en motiverende gespreksvoering. - Jeugdigen, jongvolwassenen, volwassenen en gezinnen begeleiden bij problemen op verschillende leefgebieden. - De eigen kracht van inwoners versterken. - Complexe hulpvragen analyseren om samen met inwoners vast te stellen wat nodig is. - Aanvullende specialistische hulp of regie organiseren waar nodig. - Samenwerken met scholen, huisartsen, zorgaanbieders en andere betrokken partners. - Handelen bij veiligheidsrisico's door veiligheidsafspraken te maken en passende interventies in te zetten. - Adviseren over de doorontwikkeling en positionering van het team binnen het sociale domein. Samen komen we verder In de praktijk Je werkt vanuit het lokale wijkteam Team Inzet in de gemeente Drechterland. In deze rol ondersteun je inwoners die door complexe problemen op meerdere leefgebieden tijdelijk zijn vastgelopen. Je werkt samen met inwoners en betrokken partners aan oplossingen die ook op de lange termijn standhouden. De werklocatie is Hoogkarspel; werken op kantoor heeft de voorkeur. Functie eisen - Je hebt aantoonbare ervaring binnen een sociaal wijkteam of in de ambulante jeugd- en gezinshulpverlening. - Je hebt een geldige en actuele SKJ-registratie. - Je hebt een afgeronde relevante hbo-opleiding, bijvoorbeeld Social Work, MWD of SPH. - Je hebt brede kennis van de sociale kaart, de Jeugdwet en de Wmo. - Je hebt minimaal drie jaar ervaring. - Je bent als zzp'er in staat om direct zelfstandig zaken op te pakken en je bent besluitvaardig.
