Je helpt hen groeien, ontdekken wat bij hen past en stappen te zetten richting meer zelfstandigheid. Daarbij werk je volgens het zorgplan en houd je steeds oog voor hun wensen, mogelijkheden en tempo. Door jouw begeleiding voelen cliënten zich gezien en gehoord. Juist die kleine stappen maken jouw werk waardevol. Wat ga je doen? - Je bent het aanspreekpunt voor de begeleiding van cliënten en coördineert de ondersteuning. - Je brengt samen met de cliënt de ondersteuningsvragen in kaart en vertaalt deze naar een passend zorgplan. - Je leidt cliëntbesprekingen, verzamelt relevante informatie en bewaakt de voortgang van gemaakte afspraken en acties. - Je evalueert en stelt zorgdoelen en afspraken bij waar nodig, met oog voor veiligheid, welzijn en veranderende situaties. - Je organiseert activiteiten die aansluiten bij de interesses, mogelijkheden en ondersteuningsvragen van de cliënt. De locatie Deugenweerd ligt midden in een rustige woonwijk in Borculo. Hier is plaats voor 13 volwassenen met een verstandelijke beperking. Iedere bewoner heeft een eigen appartement met een woonkamer met keuken, slaapkamer en badkamer. Daarnaast is er een gezellige gezamenlijke ruimte, waar bewoners samen kunnen eten, koffie drinken of meedoen aan activiteiten.
