Dan zoeken wij jou. Huisartsenpraktijk Harwig is een moderne en betrokken huisartsenpraktijk in Coevorden. We bieden persoonlijke, laagdrempelige en kwalitatief goede huisartsenzorg. Binnen ons team vinden we een goede werksfeer belangrijk: professioneel waar het moet, ontspannen waar het kan. Als POH-Somatiek begeleid je zelfstandig patiënten met chronische aandoeningen. Je houdt spreekuren voor onder andere diabetes mellitus, CVRM en COPD/astma. Je voert controles uit, geeft voorlichting en begeleiding, stimuleert leefstijlverandering en ondersteunt patiënten bij zelfmanagement. Waar nodig overleg je laagdrempelig met de huisarts, PA of andere collega's. In onze HAGRO werken meer praktijkondersteuners, met de andere praktijken zijn de lijntjes kort en is er ook intercollegiaal overleg mogelijk. Wat kun je van ons verwachten? - Een afwisselende functie binnen de huisartsenzorg - Zelfstandige spreekuren met veel eigen verantwoordelijkheid - Flexibel werken in overleg, passend bij de praktijk én jouw agenda - Een collegiaal team waarin humor en samenwerken belangrijk zijn - Korte lijnen en ruimte om mee te denken - Mogelijkheden voor scholing en verdieping - Salaris en arbeidsvoorwaarden conform CAO Huisartsenzorg Interesse?
