Zeno Ambulant: ZZP-opdracht: Ambulant begeleider SKJ Vacature details Solliciteren Functieomschrijving Zeno Ambulant · regio Drechtsteden/Rotterdam e.o. en regio Breda · inzet in overleg (16-36 uur p/w mogelijk) De opdracht Voor Zeno Ambulant zoeken we zelfstandige ambulant begeleiders met een SKJ-registratie voor de begeleiding van jongeren en gezinnen met een intensieve hulpvraag, in de thuissituatie. Je werkt binnen het gezinssysteem en sluit aan bij het dagelijks leven van de cliënt. Niet vanaf de zijlijn meekijken, maar aanwezig zijn: samen werken aan herstel van structuur, vertrouwen en een zinvolle daginvulling. Je kijkt verder dan het dossier en zoekt naar wat wél werkt. Je bepaalt als professional zelf hoe je de begeleiding vormgeeft, binnen de doelen die met cliënt, gezin en verwijzer zijn afgesproken. De opdracht kent een resultaatverplichting: de begeleidingsdoelen uit het plan zijn leidend, jouw vakmanschap bepaalt de route ernaartoe. Omdat we aansluiten bij het ritme van het gezin, kunnen inzetmomenten ook in avonden of weekenden vallen, in overleg en passend bij jouw agenda. Wat de opdracht inhoudt - Zelfstandig begeleiden van cliënten en gezinnen aan de hand van overeengekomen begeleidingsdoelen; - Uitvoeren en eventueel aanpassen van individuele dagprogramma's voor cliënt en gezin; - Aansluiten bij het netwerk van de cliënt: familie, school, wijkteam en andere betrokkenen; - Afstemmen met andere professionals en externe zorgpartners rondom de casus; - Signaleren en rapporteren conform de geldende kwaliteitseisen (Jeugdwet/Wmo). Wat vragen we van jou als zelfstandig professional? - Geldige SKJ-registratie harde eis; - Relevante mbo-4- of hbo-opleiding (SocialWork, SPH of vergelijkbaar); - Aantoonbare ervaring met ambulante begeleiding in gezinnen met een intensieve hulpvraag; - Ingeschreven bij de KvK, met een beroeps- en bedrijfsaansprakelijkheidsverzekering en een recente VOG niet ouder dan 3 maanden;...
