Over de functie In deze rol binnen het sociaal team van Gemeente Vijfheerenlanden werk je aan het bevorderen van de eigen kracht van inwoners en hun gezinnen. Je schakelt met verschillende werkvelden, maakt veiligheidsinschattingen, stelt actie- en veiligheidsplannen op en voert regie bij multiproblematiek. Je levert maatwerk, werkt samen met partners en speelt een rol in de doorontwikkeling van het team. Wat je gaat doen - Praktische uitvoering geven aan de Jeugdwet en schakelen met Wmo, Participatiewet en welzijnsinstellingen. - Veiligheidsinschatting maken en de ernst en aard van de zorg analyseren. - Opstellen van integrale actieplannen met ouders, jeugdigen en hun netwerk. - Opstellen van veiligheidsplannen samen met gezin en netwerk. - Werken vanuit normaliseren, niet problematiseren. - Kortdurend hulp verlenen of zorgen voor een match met partners in Welzijn of geïndiceerde zorg. - Regie voeren bij multiproblem gezinssituaties. - Veilig Thuis meldingen oppakken en opvolgen. - Bijdragen aan de doorontwikkeling van het Sociaal Team. Functie-eisen - Geldige SKJ-registratie (registratienummer vermelden in CV). - Minimaal 5 jaar aantoonbare werkervaring als professional jeugd en gezin. - Aantoonbare werkervaring met het uitvoeren van de Jeugdwet en het opstellen van ondersteunings- en veiligheidsplannen. - Afgeronde opleiding op minimaal hbo bachelor niveau in Social Work, Pedagogiek, SPH en/of MWD. - Minimaal 1 jaar aantoonbare werkervaring met veiligheidsproblematiek (bijvoorbeeld Veilig Thuis, jeugdbescherming of vergelijkbaar). - Aantoonbare werkervaring met het voeren van regie op complexe gezins- en opvoedsituaties. - Aantoonbare werkervaring me...
