Ben jij een ervaren onderwijsprofessional die sterk is in het begeleiden van leerlingen met complexe en onderbroken schoolloopbanen? Slaag je erin om onderwijs, zorg en ondersteuning rond een leerling met elkaar te verbinden? En wil je mee een nieuwe regionale werking uitbouwen die leerlingen ondersteunt bij hun terugkeer naar de thuisschool? Dan is deze functie binnen REVY misschien iets voor jou.

Vanaf schooljaar 2026-2027 start Penta met het pilootproject REVY. Binnen dit project ondersteunt een regionale trajectbegeleider leerlingen die na een periode van opname opnieuw aansluiten bij hun thuisschool. Het gaat om leerlingen die verbleven in het Zeepreventorium of in een van de vestigingen verbonden aan Penta Connect.

Als regionale trajectbegeleider vorm je de brug tussen de opnamecontext en de thuisschool. Je helpt ervoor zorgen dat opgebouwde expertise, gemaakte afspraken en handelingsgerichte adviezen niet verloren gaan bij de overgang naar de thuisschool, maar vertaald worden naar een haalbaar vervolgtraject.

Je werkt binnen een afgebakende regio in West-Vlaanderen, met Brugge, Roeselare, Kortrijk en Ieper als belangrijke ankerpunten.

REVY maakt deel uit van Penta Support, waar we expertise ontwikkelen en inzetten rond onderbroken schooltrajecten.

  • Je analyseert complexe situaties, onderscheidt hoofd- en bijzaken en vertaalt je analyse naar concrete vervolgstappen.
  • Je werkt handelingsgericht en vertrekt vanuit onderwijs- en ondersteuningsnoden van de leerling, met oog voor de mogelijkheden van de schoolcontext.
  • Je communiceert helder en verbindt leerlingen, ouders, scholen, zorgpartners en andere betrokkenen, ook wanneer verwachtingen of belangen verschillen.
  • Je neemt professioneel positie in, durft advies te geven en geeft richting wanneer een traject dreigt vast te lopen.
  • Je bent sterk in netwerken en bouwt duurzame professionele relaties op met ziekenhuisscholen, thuisscholen, CLB’s, leersteuncentra, zorgpartners en andere betrokken organisaties.
  • Je werkt zelfstandig, stelt prioriteiten en neemt verantwoordelijkheid voor de planning, opvolging en afronding van je trajecten.
  • Je kijkt verder dan individuele cases, herkent terugkerende noden en patronen en vertaalt deze naar kansen om de bredere REVY en Penta Support-werking te versterken.
  • Je bent sterk in organiseren en structureren en zorgt voor een heldere registratie, opvolging en rapportering van trajecten.
  • Je voelt je goed in een werking die nog in ontwikkeling is, neemt initiatief en helpt nieuwe werkprocessen en afspraken mee vorm te geven.
  • Je bent flexibel en bereid je frequent te verplaatsen naar scholen en partners binnen de afgebakende regio in West-Vlaanderen.
