ZorghartFlevoland Klavertje GGz Centraal zoekt een Senior Begeleider GGZ - EPA Actief werken aan het bevorderen van kwaliteit en samenwerking. Jij zorgt ervoor. Zo werk jij Als senior begeleider binnen Hofstede in Ermelo ben jij er voor klinisch opgenomen volwassenen met vaak meervoudige psychiatrische diagnoses, waarbij dubbeldiagnose ook voorkomt. De achtergronden en ziektebeelden van de doelgroep zijn zeer divers, wat het uitdagend maakt om 'op maat' herstel-ondersteunende behandeling te bieden. Je stimuleert een gezonde leefstijl en werkt met cliënten aan hun herstel vanuit de ART-methodiek. Binnen het team vervul je een voorbeeldfunctie en werk je actief aan het bevorderen van kwaliteit en samenwerking. Vanuit jouw expertise en persoonlijke talenten geef jij vorm aan de functie van ggz-agoog/ senior begeleider. Als ggz-agoog/ senior begeleider kun je de zorg nog beter afstemmen op de patiënten en de teamsamenwerking optimaliseren. Wat de functie van ggz-agoog/ senior begeleider bij GGz Centraal bijzonder maakt is: - Het coachen van cliënten in hun herstel. Vanuit het behandelplan begeleid je cliënten bij het nemen van eigen regie en het vinden van zingeving in hun leven. - Methodisch werken. Zowel richting cliënten als binnen het team ga je systematisch en planmatig te werk, met oog voor kwaliteit en continuïteit. Op het gebied van coördinatie, kwaliteit en zorgvernieuwing neem je het voortouw in: - Je coacht collega's en zorgt voor doorontwikkeling van het team. Je weet het reflectief en lerend vermogen binnen jouw team te bevorderen en pakt actief een voorbeeldrol. - Je houdt je actief bezig met innovatie, digitalisering en kwaliteitsbevordering. Je initieert, implementeert en borgt verandertrajecten. Ook denk je mee over beleid en processen, signaleer je knelpunten en neem je deel aan projecten. - Je hebt meerdere jaren ervaring met patiënten met complexe en/of meervoudige psychiatrische problematiek.
