PV Neuzeug-Sierning sucht für das Gesundheitszentrum 1 Sozialarbeiter/in Beschäftigungsausmaß: Teilzeit, 15 Wochenstunden

Dienstbeginn: ab sofort

Diese Tätigkeit erfolgt in enger Zusammenarbeit mit weiteren Gesundheitsberufen im Team. Das Kernteam, bestehend aus Ordinationsassistenz, diplomierten Pflegekräften und ÄrztInnen, wird von einem erweiterten Team mit Expertise aus den Bereichen Diätologie, Sozialarbeit, Ergotherapie, Physiotherapie, Psychologie und/oder Psychotherapie, professionelles Wundmanagement sowie Pharmakologie ergänzt.

Ziel ist eine hochqualitative Betreuung unserer PatientInnen durch interdisziplinäre Zusammenarbeit sowie regelmäßige Fallbesprechungen und Teambesprechungen mit allen im Primärversorgungszentrum vertretenen Gesundheitsberufen.

Ihre Hauptaufgaben

* Beratung und Hilfestellung für PatientInnen sowie deren Bezugspersonen

* Schnittstellenkoordination innerhalb des Versorgungssystems

* Interdisziplinäre Besprechungen

* Hausbesuche und persönliche Begleitungen nach Erforderlichkeit

* Mitwirkung an Veranstaltungen zur Gesundheitsförderung und Prävention

Unsere Anforderungen

* Abgeschlossene Berufsausbildung (Diplom, Bachelor, Master oder FH Soziale Arbeit)

* Kenntnisse des regionalen Sozialnetzwerks

* Berufserfahrung erwünscht

* Hohe soziale Kompetenz und professionelles Auftreten

* Hohe Eigenmotivation

* Teamfähigkeit und Freude an der Arbeit mit Menschen

* Hohe Organisationsfähigkeit sowie Bereitschaft zum eigenverantwortlichen Arbeiten

Entlohnung

Für diese Stelle bieten wir ein monatliches Bruttogehalt von mindestens € 2.874,10 auf Vollzeitbasis. Eine Überzahlung ist - abhängig von Ihrer beruflichen Qualifikation und Erfahrung - möglich.
