Wij zoeken een casemanager die aan de slag gaat met de complexe casuïstiek en het beoordelen van toeslagen voor extra zorg binnen de Wet langdurige zorg (Wlz). Wat ga je doen? Als Verpleegkundig casemanager Wlz bij het zorgkantoor Zorg en Zekerheid werk je midden in de dynamiek van de langdurige zorg. Geen dag is hetzelfde. Je houdt je bezig met complexe casuïstiek en toeslagen extra zorg; Complexe casuïstiek Je behandelt uiteenlopende, vaak gevoelige en ingewikkelde zorgsituaties die bij ons binnenkomen via zorgaanbieders, gemeenten, het veiligheidshuis, crisisregisseurs, cliëntvertegenwoordigers of wettelijke vertegenwoordigers. Je onderzoekt de situatie en analyseert wat er écht nodig is, schakelt met betrokken partijen en weegt belangen zorgvuldig af, brengt helderheid in moeilijke situaties en komt - soms onder tijdsdruk - tot een advies en/of besluit. Dat vraagt doorzettingsvermogen, overzicht en goede communicatie. Maar het geeft ook veel energie omdat jij, in de samenwerking met betrokkenen, cliënten en hun netwerk weer perspectief kan bieden. Toeslagen extra zorg Daarnaast beoordeel je aanvragen voor toeslagen extra zorg zowel voor het persoonsgebonden budget (pgb) als Zorg in Natura (ZIN). Je onderzoekt welke extra zorg een cliënt nodig heeft en wat hiervoor passend en verantwoord is. Dit doe je niet alleen achter je bureau, maar ook door in gesprek te gaan met bijvoorbeeld wettelijke vertegenwoordigers, managers, gedragsdeskundigen, begeleiders of artsen. Ongeveer 25% van je tijd ga je met je directe collega op locatie- en/of huisbezoek om de aanvraag te bespreken en een beeld te krijgen van de woonomgevi...
