Dit ga je doen Als medewerker van het Meldpunt zorgwekkend gedrag kom je in actie wanneer er zorgen zijn om iemand en het niet lukt om op de gebruikelijke manier hulp te bieden. Je krijgt meldingen over mensen die de grip op hun leven kwijt zijn, zorg mijden of zichzelf en hun omgeving ernstig verwaarlozen. Jij brengt in beeld wat er speelt, schat de risico's in en zorgt dat er beweging komt in de situatie. Daarmee help je voorkomen dat situaties verder escaleren en zorg je dat mensen de juiste hulp krijgen. Je neemt meldingen aan en onderzoekt wat er aan de hand is. Je beoordeelt de situatie, geeft advies en stemt af met de betrokken organisaties. Ook leg je contact met de inwoner en ga je waar nodig op huisbezoek om de situatie goed te kunnen beoordelen en pass...
