Hierdoor blijf ik bezig met mijn persoonlijke ontwikkeling." Jouw rol als WMO Consulent: Jij maakt het verschil. Dankzij de zorg die jij vanuit de gemeente toekent, kunnen inwoners langer en fijner zelfstandig blijven wonen, zich beter verplaatsen en de begeleiding krijgen die zij nodig hebben. Denk hierbij bijvoorbeeld aan woningaanpassingen, trapliften, scootmobielen, taxipassen, huishoudelijke ondersteuning als het gaat om fysieke beperkingen. Inwoners met mentale uitdagingen kunnen bij jou terecht als het gaat om aanvragen voor dagbesteding of individuele begeleiding. Daarnaast stem je in deze rol zorg- en ondersteuningstrajecten direct af met zorgaanbieders en maatschappelijke organisaties. Je bewaakt de kwaliteit en voortgang van de hulpverlening en zorgt voor een soepele samenwerking tussen alle betrokken partijen. Je komt te werken bij een gemeente in de buurt van Amsterdam in het team WMO, of je wordt met een geheel BMC projectteam ingezet om een gemeente volledig te ontzorgen en achterstanden kwalitatief weg te werken.
