Woonbegeleider - - Almere - € 2.888 - € 3.917 per maand - Team Outreach Almere Functieomschrijving Gewoon Thuis biedt cliënten zowel geplande als ongeplande ondersteuning in hun eigen thuissituatie. Dit gebeurt in samenwerking met zorgpartners, voorzieningen en het sociale netwerk, zodat cliënten sneller en langer zelfstandig kunnen wonen. Als woonbegeleider ben jij de spil in de zorg rondom de cliënten. Je ondersteunt hen bij dagelijkse zaken, helpt hen bij het nastreven van hun doelen en schakelt actief met de persoonlijk begeleider(s). Je kunt meegaan naar bijvoorbeeld gesprekken bij de huisarts, gemeente of bewindvoerder ter ondersteuning van de cliënt. Daarnaast worden verschillende vormen van zorg ingezet, zoals trajectbegeleiding, budgetcoaching en zorg op afstand via beeldbellen. Ook hierbij kun je ondersteuning bieden of gesprekken samen met de cliënt voorbereiden. Wij zoeken een flexibele en gedreven collega die stevig in zijn of haar schoenen staat, empathisch is, zelfstandig kan werken én goed kan samenwerken. Je biedt zowel geplande als ongeplande zorg, maakt deel uit van de bereikbaarheidsdienst (inclusief avonden en incidenteel weekenden) en kunt bij intensieve situaties terugvallen op het team en het achtervangnet. De cliënten hebben vaak te maken met complexe of psychiatrische problematiek, waaronder een psychotische kwetsbaarheid. Je beschikt over basiskennis en een theoretische achtergrond, kunt situaties goed inschatten, signalen herkennen en hier professioneel naar handelen. Kortom: jij bent de verbindende factor die cliënten ondersteunt in hun zelfstandigheid, met oog voor hun eigen regie en passende zorg op maat. Jouw werkplek Gewoon Thuis Almere begeleidt ongeveer 35 cliënten. Bij Gewoon Thuis werk je in een subteam van 5 medewerkers en daarnaast zijn wij onderdeel van Team Outreach Almere, dat uit ongeveer 20 zorgprofessionals bestaat.
