Je komt te werken in Clinge op twee woonlocaties waar cliënten wonen met een matig verstandelijke beperking en moeilijk verstaanbaar gedrag. Je helpt de cliënten om zoveel mogelijk zelf keuzes te maken en eigen regie te ervaren. Soms betekent dit dat je de client stimuleert en ondersteunt en soms dat je juist duidelijkheid, structuur en nabijheid biedt. Belangrijk is telkens af te stemmen op de individuele behoeften van elke client. Je past een gehechtheid bevorderende begeleidingsstijl toe, ook als dit minder vanzelfsprekend is door moeilijk verstaanbaar gedrag. Als zorgcoördinator ben je verantwoordelijk voor de coördinatie en kwaliteit van zorg. Je bent sensitief voor veranderingen in gedrag, welzijn en ondersteuningsbehoeften van de cliënten en stemt met alle betrokkenen af hoe jullie de zorg het beste laten aansluiten. Je bent als zorgcoördinator teamcoach van het team, waarbij je het team coacht op zorginhoudelijke aspecten en de teamdynamiek, en de persoonlijke begeleiders ondersteunt in de uitvoering van de coördinerende en coachende rol. Je werkt wekelijks mee in het rooster, samen met je team. Een veilige, stabiele en voorspelbare leefomgeving voor cliënten is telkens het uitgangspunt. Hierbij is het opbouwen en borgen van een onvoorwaardelijke ondersteuningsrelatie essentieel. Daarnaast maak je deel uit van het zorgcoördinatorteam van het cluster MVB II, waarbij je participeert in de team overleggen en je collega ondersteunt in de dagelijkse praktijk in het cluster MVB II.
