The posting

Als zorgcoördinator sta je mee in voor het uitbouwen en opvolgen van een krachtig zorgbeleid binnen onze school. Je vormt een belangrijke schakel tussen leerlingen, ouders, leerkrachten, leerlingenbegeleiding en externe partners.

Je ondersteunt leerlingen die extra begeleiding nodig hebben en zorgt ervoor dat de onderwijs- en ondersteuningsbehoeften van leerlingen tijdig worden opgevolgd. Daarbij werk je nauw samen met het schoolteam en denk je actief mee over een verbindend en inclusief schoolbeleid.

Je takenpakket

  • Je coördineert en organiseert de leerlingenbegeleiding binnen de school.
  • Je brengt de onderwijs- en ondersteuningsbehoeften van leerlingen in kaart en volgt deze verder op.
  • Je begeleidt leerlingen individueel of in kleine groepen wanneer extra ondersteuning nodig is.
  • Je overlegt met leerkrachten en ondersteunt hen bij het omgaan met specifieke onderwijs- en zorgbehoeften.
  • Je onderhoudt contacten met ouders en externe partners, zoals het CLB en andere hulpverleningsinstanties.
  • Je volgt zorgdossiers en gemaakte afspraken zorgvuldig op.
  • Je neemt deel aan overlegmomenten en klassenraden en draagt relevante informatie bij aan het zorgbeleid.
  • Je werkt mee aan de verdere ontwikkeling en implementatie van het zorgbeleid van de school.
  • Je signaleert noden en denkt mee na over passende oplossingen en begeleidingsmaatregelen.
  • Je draagt bij aan een veilig, positief en inclusief schoolklimaat.

Je bent iemand die:

  • leerlingen op een laagdrempelige en respectvolle manier kan benaderen;
  • rustig en doordacht handelt, ook in moeilijke situaties;
  • verschillende belangen kan verbinden;
  • sterk is in plannen, organiseren en opvolgen;
  • duidelijk en constructief communiceert met leerlingen, ouders en collega's;
  • initiatief neemt en graag meedenkt over de verdere uitbouw van de school;
  • flexibel kan inspelen op veranderende situaties en noden.
  • Een bachelordiploma;
  • Sterke communicatieve en organisatorische vaardigheden;
  • Een groot empathisch vermogen en een luisterende houding;
  • Het vermogen om zelfstandig te werken én vlot samen te werken binnen een team;
  • Discretie en een professionele omgang met vertrouwelijke informatie;
  • Een probleemoplossende en proactieve ingesteldheid;
  • Affiniteit met jongeren en het secundair onderwijs.
