Vanaf 2025 wordt het team van cardiologen binnen AZ Oostende versterkt door een Hartfalencardioloog. Om deze samenwerking uit te bouwen zoeken we een Hartfalenverpleegkundige op de polikliniek cardiologie (hartfalenkliniek). Je vervult binnen jouw specialisatiedomein drie kerntaken: rol van Expert – Educator/Coach en Kwaliteitspromotor

Expert

- Je informeert en licht de patiënten en familie/mantelzorger in over het specifieke ziektebeeld - Je bepaalt samen met de patiënt haalbare doelen en begeleidt ze in het verhogen van de autonomie - Je organiseert een spreekuur in nauwe samenwerking met de arts(en) - Je bent centraal aanspreekpunt (consultfunctie) en verwijst indien nodig door naar andere deskundigen - …
