Sous l'autorité du responsable du SPASAD et de l'infirmière coordinatrice, l'infirmier assume la responsabilité des soins infirmiers du SSIAD/SPASAD. L'infirmier(e) dispense, planifie et organise les soins de nature technique, relationnelle et éducative. Il participe au travail d'évaluation, aux diagnostiques infirmiers et à l'élaboration des projets de soins individualisés. (selon le décret de compétence de la profession infirmier: décret n°2016-1605 du 25 novembre 2016)

MISSIONS/ACTIVITES :

  • Contribuer au recueil d'informations et à l'évaluation de la personne à prendre en soins.
  • Évaluer les niveaux de dépendance.
  • Mettre en place en collaboration avec les aides-soignants, les plans de soins et la planification des interventions.
  • Dispenser ou superviser des actes et/ou soins sur rôle propre ou prescription.
  • Garantir le respect des règles d'hygiène et différents protocoles. Sous l'autorité du responsable du SPASAD et de l'infirmière coordinatrice, l'infirmier assume la responsabilité des soins infirmiers du SSIAD/SPASAD.
  • Evaluer, informer et préconiser les aides ou aménagements nécessaires au maintien au domicile
  • Contribuer à informer et accompagner les personnes soignées, les aidants et les auxiliaires de vie, pour l'éducation et la prévention des pathologies.
  • Transmettre à la responsable de service et à l'infirmier(e) coordinatrice(teur), les informations concernant l'évolution de l'usager
  • Encadrer, accompagner les AS autour d'analyse de pratique ou de réflexion éthique.
  • Assurer et superviser les outils de transmissions.
  • Utiliser le logiciel de transmissions, mettre en place les dossiers de soins.
  • Travail en collaboration entre le SSIAD et le SAAD.
  • Communiquer avec les partenaires extérieurs du secteur médico-social.
  • Assurer le fonctionnement quotidien du service en l'absence de l'IDEC (prise en compte des demandes, évaluation à domicile, gestion du planning...).
  • Formation, encadrement des stagiaires ou nouveaux membres du personnel.

CONDITIONS/CONTRAINTES DE TRAVAIL :

  • Horaire continu ou en coupure, 1 WE / 2 travaillé, jours fériés.
  • En continu : 08h/12h45 et 13h30/15h45
  • En coupure 08h/12h30 et 16h30/20H00
  • Planning mensuel révisable selon besoin de service.

DEGRE D'AUTONOMIE : grande autonomie

NIVEAU DE RESPONSABILITE : moyen

DILPÔMES/FORMATIONS/HABILITATIONS :

Etre titulaire du diplôme d'Etat infirmiers/Permis B obligatoire

SAVOIRS :

  • Connaître les pathologies liées au veillissement ou au handicap.
  • Maîtriser les soins techniques, relationnels et éducatifs.
  • Respecter les règles d'hygiène et les protocoles.
  • Connaître les dispositifs d'aide du maintien au domicile.
  • Appliquer les gestes de premier secours et de protection aux personnes.
  • Concevoir et conduire un projet de soins infirmiers.
  • Respecter les véhicules et outils de travail mis à dispossition et en assurer l'entretien.
