Bienvenue au sein de la Fondation Œuvre de la Croix Saint-Simon.

Acteur majeur dans les secteurs sanitaires et médico-sociaux aux services des personnes vulnérables de la naissance à la fin de vie.

Notre projet institutionnel est de proposer une approche globale et diversifiée d’accueil, d’accompagnement, de soins ou de prise en charge médicale en Ile-de-France.

Fortement impliqué dans le territoire de l’Est Parisien, l’équipe du SSIAD de la fondation, participe au maintien à domicile des personnes âgées en situation de handicap. Il propose une prise en charge des soins de la vie quotidienne et des soins infirmiers en favorisant la proximité avec les patients et leurs aidants et en recherchant la valorisation de l’autonomie.

Notre pôle est engagé dans une dynamique de synergies : Service d’Accompagnement Médico-Social pour Adultes Handicapés (SAMSAH), Centre Accueil de Jour (CAJ), Hospitalisation à Domicile (HAD), EHPAD (accueil temporaire d’urgence, PASA et UVP). Il porte des projets innovants en termes d’expérimentations de nouvelles approches thérapeutiques non médicamenteuses.

Le SSIAD offre une qualité de travail : prises en charge en binôme (IDE, AS), regards croisés avec ergothérapeute, psychomotricien, assistante sociale, psychologue et une disponibilité accrue.

Vous serez intégré(e) au sein d’une équipe pluridisciplinaire bienveillante et à l’écoute.

Vos principales missions sont :

Sous la responsabilité de la directrice, vous êtes chargé(e) de :

  • Réaliser les visites à domicile des usagers du SSIAD : accompagnement et soutien psychologique auprès de personnes âgées de plus de 60 ans, évaluation globale des conditions de vie et des besoins, des capacités cognitives et sociales, de l’humeur, du risque suicidaire, entretiens cliniques et séances de relaxation (sophrologie, musicothérapie, toucher relationnel).
  • Effectuer les transmissions des synthèses sur le logiciel de soin Domilink Mobisoins.

Auprès des aidants familiaux :

  • Evaluaer des situations de vie dans leur globalité, entretiens à visée de soutien individuel et/ou aidant(s)-aidé, séances de relaxation individuelles et collectives.
  • Création et co-animation avec l’ergothérapeute et la psychomotricienne d’un groupe d’entraide à destination des aidants, organisation de réunions thématiques à visée d’information et de soutien par le partage d’expériences
  • Sooliciter les professionnels soignants du SSIAD en fonction des problématiques rencontrées aux domiciles, de l’histoire de vie des usagers et des connaissances des troubles cognitifs et des comportementaux liés aux maladies neuroévolutives.
  • Travailler en réseau avec le DAC Paris Human’est, le CASVP, Pallidom pour l’accompagnement global des usagers.

Vous avez

· Un diplôme en psychologie en psychopathologie du vieillissement ou en neuropsychologie clinique.

· Connaissance du public âgé atteinte de maladies neuroévolutives ou en perte d’autonomie.

· Le sens des responsabilités et de l’organisation.

· Un esprit curieux et une personnalité dynamique.
