Job Title: RN Case Manager | Southwest Minneapolis Home Health | $6,000 First-Year Bonus Company: Home Health Care, Inc. Location: Edina, St. Louis Park, Richfield, Hopkins & Southwest Minneapolis Schedule: Full-Time | Monday through Friday, 8:00 AM to 5:00 PM | 1 Weekend per Month | 1 On-Call Shift per Week Compensation: $85,000 to $130,000 annually + $6,000 in First-Year Bonuses To Stand Out Email Elena Ehrlich at [email protected] with your resume and a brief overview of your RN experience, clinical background, OASIS or home health experience, and what you're looking for in your next nursing position. What If They Were Actually Your Patients? Not the patients assigned to you for today's shift. Not the patient in Room 312 who someone else will take over tonight. Your patients. Your caseload. Your responsibility to help move their care forward. That's one of the biggest differences between traditional bedside nursing and working as an RN Case Manager in home health. You aren't simply stepping into someone's care for eight or twelve hours. You're getting to know them. You're following their progress. You're coordinating with their physicians and therapists. You're educating their families. You're recognizing when something changes. And you're helping guide their care from admission through discharge. If you've ever wanted more ownership of your nursing, this is it. Know More Than What's Happening Today Great Case Managers know their patients. You know the patient who struggles to manage their medications. You know which caregiver needs a little more education. You know which wound should be progressing faster. You know which patient recently changed medications. You know who's making progress with therapy. And because you're following that patient over time, you recognize when something doesn't look right. That continuity gives you the opportunity to intervene earlier, communicate concerns, adjust the plan of care, and help prevent avoidable hospitalizations. You're not simply completing today's visit. You're thinking about what needs to happen next. Your Territory: Southwest Minneapolis & Inner Metro You'll manage patients within a concentrated area of the southwest Minneapolis and inner-ring metro, including: Edina, St. Louis Park, Richfield, Hopkins, and Southwest Minneapolis, including areas within the 55410 and 55416 ZIP codes. Patient scheduling and territory assignments are designed around geographic needs, with visits generally concentrated within approximately a 35-mile radius of your home whenever possible. What You'll Own As an RN Case Manager, you'll oversee an assigned home health caseload and coordinate care throughout each patient's episode. You'll: Manage an assigned home health patient caseload Complete OASIS Start of Care, Recertification, and Discharge assessments Perform comprehensive nursing assessments Develop and update individualized plans of care Provide skilled nursing visits Monitor patient progress throughout the episode of care Perform wound care, medication management, and other skilled nursing services Educate patients and caregivers on diagnoses, medications, treatments, and recovery Identify and communicate changes in patient condition Coordinate directly with physicians and healthcare providers Collaborate with PT, OT, SLP, social work, and interdisciplinary team members Help patients safely transition home following hospitalization or rehabilitation Work proactively to prevent avoidable emergency department visits and rehospitalizations Complete timely documentation using Homecare Homebase (HCHB) You're Still a Nurse. You Just Get to See More of the Story. Case Management doesn't mean leaving clinical nursing behind. You're still assessing patients. Providing skilled care. Managing wounds. Evaluating medications. Educating families. Recognizing changes in condition. But now you're also connecting those individual visits into a larger clinical picture. Where was this patient when we started? Where are they today? What's preventing them from reaching their goals? What needs to happen next? That's where your experience and clinical judgment become incredibly valuable. Why Nurses Choose Home Health Case Management $85,000 to $130,000 annually $6,000 in first-year bonuses Full-Time employment Primarily Monday-Friday schedule Concentrated Southwest Minneapolis & Inner Metro territory Manage your own patient caseload Build ongoing relationships with patients and caregivers Meaningful 1:1 skilled nursing care Greater ownership of the patient's overall plan of care Blend hands-on nursing with clinical leadership and care coordination Independence in the field with an experienced clinical team behind you Homecare Homebase (HCHB) documentation Join an established home health organization with more than 30 years of experience Who We're Looking For The best Case Managers aren't simply strong nurses. They take ownership. They're organized enough to manage a caseload. Clinical enough to recognize when something is changing. Confident enough to call the physician. Patient enough to educate a family. And accountable enough to make sure the next step actually happens. We're looking for: An active Minnesota Registered Nurse (RN) license Three or more years of RN experience Home health experience preferred OASIS experience highly valuable Strong comprehensive assessment and critical-thinking skills Excellent organization and time management Confidence working independently in patients' homes Strong patient, caregiver, physician, and interdisciplinary communication Reliable transportation for visits throughout your assigned territory Experienced nurses from hospital, acute care, rehabilitation, skilled nursing, and other clinical environments are encouraged to connect with us. About Home Health Care, Inc. Since 1994, Home Health Care, Inc. has provided skilled home health services to patients throughout Minnesota. As an established, locally owned Medicare-certified home health agency, we've spent more than three decades helping patients recover safely, manage complex medical conditions, and maintain their independence at home. And our RN Case Managers are at the center of making that happen. Stop Taking Care of a Patient for a Shift. Start Helping Lead Their Recovery. Know their story. Understand their goals. Coordinate their care. Recognize their progress. Catch the setbacks. Celebrate the discharge. That's RN Case Management. If you're ready for more ownership of your patients and want to see what happens beyond today's shift, we'd like to meet you. $85,000 to $130,000 annually. $6,000 in first-year bonuses. Edina, St. Louis Park, Richfield, Hopkins & Southwest Minneapolis. Email Elena Ehrlich at [email protected] with your resume and a brief overview of your RN experience. Your caseload. Your patients. Your opportunity to make an impact from start to finish.

Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
