Home Health Care, Inc. is looking for a Full-Time RN Visit Nurse to provide skilled nursing care throughout Mankato and surrounding Southern Minnesota communities. Your patients need care. You're seeing how they're actually managing at home. Is the medication list accurate? Is the wound healing? Does the patient understand the discharge instructions? Can the caregiver safely help with the treatment plan? Has something changed since the patient left the hospital? Is there an issue that could turn into an emergency if nobody catches it today? You get to see the whole picture. And then you get to use your nursing judgment to do something about it. What You'll Be Doing Provide skilled RN home health visits throughout your assigned territory Complete comprehensive nursing assessments Perform wound assessment and wound care Provide medication reconciliation and management Administer injections and other skilled nursing treatments Monitor acute and chronic medical conditions Identify and respond to changes in patient condition Educate patients and caregivers about medications, diagnoses, treatments, and recovery Coordinate care with physicians and healthcare providers Collaborate with PT, OT, SLP, social work, and interdisciplinary team members Support patients transitioning home following hospitalization or rehabilitation Help reduce avoidable emergency department visits and hospital readmissions Complete timely documentation using Homecare Homebase (HCHB) Home Health Needs Nurses Who Can Think for Themselves. You'll need: An active Minnesota Registered Nurse (RN) license Previous experience in home health, hospital, acute care, rehabilitation, skilled nursing, or another clinical environment Strong assessment and critical-thinking skills Confidence making clinical decisions in the field Excellent patient and caregiver communication Strong organization and time-management abilities Ability to manage a daily schedule of patient visits Reliable transportation and willingness to travel throughout the Southern Minnesota service area Previous home health experience is certainly valuable, but it isn't the only background that can prepare you for this position.
