Job Title: Physical Therapist Assistant (PTA) | Home Health Rehabilitation Company: Home Health Care, Inc. Location: Western Twin Cities Metro & Buffalo Area | Delano, Buffalo, Mound, Watertown, Cokato & Surrounding Communities Schedule: Full-Time | Weekend Rotation Compensation: $70,000–$102,000 per year (Paid Per Visit) + Mileage Reimbursement To Stand Out Email Nikky Vogelgesang at [email protected] with your resume and a brief overview of your PTA experience, clinical background, and what you're looking for in your next Physical Therapy position. Your Work Should Have Room to Pay Off. You've built the clinical skills. You know how to motivate patients. You know how to keep someone progressing when recovery gets difficult. And you know how to manage a productive day. So why work in a therapy position where doing more rarely changes what you earn? As a Physical Therapist Assistant with Home Health Care, Inc., you'll work in a per-visit compensation model with annualized earning potential of $70,000–$102,000. Your day happens in the field. Your patients receive your full attention. And your productivity can directly contribute to what you earn. More Independence. More Ownership. More Earning Potential. Home health isn't about sitting in the same therapy gym waiting for your next patient to arrive. You'll be out in the community managing a field-based schedule and providing one-on-one Physical Therapy treatments in patients' homes. You'll work from an established plan of care, collaborate closely with the supervising Physical Therapist, and help patients make measurable progress toward greater mobility and independence. And because you're paid per visit, your productivity matters. For an organized, motivated PTA who knows how to manage a schedule, that creates an opportunity to build a strong career in home health. $70,000–$102,000 in annualized earning potential. Paid per visit. Mileage reimbursement between patient visits. And a team behind you while you're working independently in the field. Your Territory: Western Metro & Buffalo Area We're hiring a PTA to serve patients throughout the Western Twin Cities Metro and communities extending toward Buffalo and surrounding areas. Scheduling is coordinated geographically whenever possible to help cluster patient visits and create a more efficient field schedule. What You'll Actually Be Helping Patients Accomplish Your patients aren't coming to you because they want to become better at therapy exercises. They want their lives back. They want to: Walk safely through their home Get out of bed independently Stand up from their favorite chair Navigate stairs Improve their balance Reduce their risk of falling Regain strength following hospitalization or surgery Safely use a walker or other assistive device Get back outside Rely less on family and caregivers Your treatments are what help turn those goals into progress. What You'll Do Working under the direction of the supervising Physical Therapist, you'll: Provide one-on-one Physical Therapy treatments in patients' homes Carry out individualized plans of care established by the Physical Therapist Provide therapeutic exercise and strengthening Perform gait, balance, transfer, and mobility training Help improve endurance, coordination, and functional independence Reinforce home exercise programs Educate patients and caregivers on safe mobility and therapeutic techniques Monitor patient response to treatment Recognize and communicate changes in condition Provide progress updates to the supervising Physical Therapist Collaborate with nursing, OT, SLP, physicians, and interdisciplinary clinicians Complete timely and accurate clinical documentation The Right PTA Can Thrive in a Per-Visit Model Home health rewards a different kind of therapist. Someone who is organized. Someone who communicates. Someone who can independently manage their day. Someone who keeps patients engaged and progressing. Someone who understands that efficiency doesn't mean rushing care—it means knowing how to run a great schedule. If that's you, the per-visit model can create an opportunity to combine independence with strong earning potential. Why PTAs Choose Home Health Care, Inc. $70,000–$102,000 annualized earning potential Paid per visit Mileage reimbursement between patient visits Full-Time employment Meaningful one-on-one patient care Greater independence throughout your workday Ability to manage a field-based schedule No traditional outpatient clinic environment Ongoing collaboration with experienced Physical Therapists Interdisciplinary clinical support Opportunity to build ongoing relationships with patients throughout recovery Benefits That Go Beyond Your Visit Rate We also provide: Free access to our MedBridge Education Hub for continuing education and professional development Licensure reimbursement when it's time to renew Annual scrub reimbursement Mileage reimbursement between patient visits Cell phone reimbursement for eligible clinicians Comprehensive medical, dental, and vision coverage for eligible employees PTO Paid holidays 401(k) Structured orientation and ongoing clinical support Career opportunities throughout our family of healthcare companies Never Worked in Home Health? That's Okay. Your PTA experience doesn't have to come from another home health agency. Experience in: Outpatient Physical Therapy Skilled nursing TCU Inpatient rehabilitation Orthopedics Assisted living Neurological rehabilitation Other direct patient-care environments can translate well into home health. You bring the therapy foundation. We'll help you learn how to take it into the field. Why it works

3×

more interviews than doing it yourself with ChatGPT.

ChatGPT writes a CV and never learns what happened to it. We see every reply. For each CV we know:

  • How it was written, and how the review scored it
  • When we applied, and how long after the posting went up
  • Which posting, which company, which city
  • Who got the interview, and who heard nothing

That is how we know which CVs get called.
