Afin de renforcer nos équipes déjà en poste, sur diverses structures genevoises et vaudoises (La Côte), nous recherchons des Assistant\-e\-s en Soins et Santé Communautaire Assistant en soins et santé communautaire ASSC \- avec CFC (H/F) Secteurs : Hospitalier \- EMS \- Soins à Domicile sur les cantons de Genève et de Vaud (La Côte) Votre mission : Assurer la prise en charge globale des patients ou bénéficiaires (soins techniques et soins de confort) ; Réaliser des soins personnalisés et de qualité au sein d'équipes pluridisciplinaires ou en autonomie à domicile ; Effectuer la traçabilité des soins et assurer la transmission des informations essentielles à la continuité de la prise en charge ; Participer au maintien de l'autonomie et au bien\-être des personnes accompagnées ; Intervenir dans le cadre de missions ponctuelles, remplacements planifiés ou urgents, ainsi que de missions au pied levé selon les besoins ; S'adapter à différents environnements de travail : établissements de santé, structures médico\-sociales ou interventions à domicile. Votre profil : Certificat fédéral de capacité (CFC) exigé ; Droit de pratique valable sur le canton concerné ou reconnaissance en cours ; Vous êtes polyvalent(e), flexible et autonome ; Vous appréciez le travail en équipe tout en étant capable d'intervenir de ère indépendante ; Vous disposez d'une excellente aisance relationnelle et d'un sens développé du service ; Vous savez vous adapter rapidement à de nouveaux environnements de travail ; Vous êtes disponible pour des missions régulières ou ponctuelles selon vos disponibilités ; La mobilité représente un atout, notamment pour les interventions à domicile.
