L-Hôpital Riviera-Chablais, Vaud-Valais (HRC), est un hôpital intercantonal de référence, qui réunit plus de 2-000 collaboratrices et collaborateurs. Le Centre hospitalier de Rennaz, compte plus de 350 lits de soins aigus. Il est complété par deux Cliniques de gériatrie et réadaptation ainsi que deux permanences médicales. L-ensemble des collaboratrices et collaborateurs s-investissent au quotidien pour fournir une prise en charge de qualité aux patient·e·s, en incarnant les valeurs de l-institution : l-authenticité, l-exemplarité et l-innovation.

Son Service de Gériatrie et Réadaptation SGR offre ses prestations sur 3 sites, à savoir :

  • La Clinique de Gériatrie et Réadaptation CGR de la Riviera à Vevey (site de la Providence)
  • La Clinique de Gériatrie et Réadaptation CGR du Chablais à Monthey
  • Le site de Rennaz.

Les CGR ont pour mission la réadaptation principalement gériatrique, ainsi que les soins aigus aux séniors. À Rennaz il s-agit de consultations de Gériatrie hospitalières (CGH).

Une collaboration étroite avec l-HRC permet à nos patients de bénéficier de son plateau technique et de ses équipements de dernière génération.

Sous la hiérarchie d-un binôme médico-soignant, vous travaillerez en étroit partenariat avec le patient et ses proches, tout en mettant l-accent sur une collaboration interdisciplinaire.

Le Service de Gériatrie et Réadaptation recherche des :

Assistants en soins et santé communautaire (ASSC) à 80%-100%

Les missions principales de ce poste sont ###

  • Organiser, exécuter, surveiller et documenter les prestations de soins et les actes médico-techniques délégués par l'infirmier et autorisés par le statut d-ASSC, dans les situations stables ne présentant aucun caractère d'urgence
  • Collaborer au bon déroulement des activités du service
  • Promouvoir l-approche interdisciplinaire des soins et le travail en équipe spécifique à la mission du service
  • Promouvoir, sur délégation, l-implication du patient dans son projet de soins ainsi que celui de ses proches

Nous demandons ###

  • Impérativement un CFC d-assistant-e en soins et santé communautaire
  • Une personne motivée à prendre en charge les seniors selon les bonnes pratiques de la Gériatrie et de la Réadaptation
  • Une personne rigoureuse, organisée et dynamique, avec d-excellentes compétences en communication
  • Un intérêt à travailler en équipe interdisciplinaire
  • Une adaptation au changement et une implication dans le projet du service
  • Une maîtrise de la langue française et des outils informatiques spécifiques au domaine hospitalier
