Promoteur des conditions de succès à l’échelle locale et au plus près de la population, le Service du Développement des Pratiques Professionnelles (SDPP) de l’AVASAD répond à des enjeux de qualité, d’accompagnement métier, de développement des capacités et d’innovation dans le domaine du maintien à domicile, notamment en matière de prévention et promotion de la santé. Cette équipe de 30 collaboratrices et collaborateurs est la garante d’une offre évolutive et équitable pour l’ensemble de la population vaudoise. Pour compléter l'équipe, nous sommes à la recherche d'un·e :

Assistant·e social·e (70% à 90%)

Missions et responsabilités :

  • Prendre en charge les demandes arrivant à la centrale des solidarités. Cette ligne téléphonique a pour but d’offrir une réponse coordonnée ainsi que de promouvoir l’action sociale sur l’entier du Canton ;
  • Apporter une expertise et assurer une réponse sociale de qualité conformément au code de déontologie définie par la profession ;
  • Établir un plan d’intervention de court, moyen et long terme dans les situations le nécessitant ;
  • Collaborer avec les partenaires du réseau socio\-sanitaire ;
  • Contribuer à la création et l’évolution de la documentation interne pour garantir la qualité des prestations de la Centrale des Solidarités.

Vos compétences et votre expérience :

Nous recherchons une collaboratrice, un collaborateur qui dispose idéalement d’une expérience de 1 à 3 années de pratique de la prise en charge sociale. La/le candidat·e idéal·e a des connaissances du milieu médico\-social.

La fonction nécessite :

  • Un Bachelor d’assistant·e social·e HES
  • Un fort intérêt pour le travail en équipe tout en ayant une grande capacité à travailler en autonomie
  • Un sens du détail avec une capacité d’analyse, tout en étant orienté·e solutions, clients et résultats
  • Une personnalité ouverte, communicative, collaborative avec de l’aisance dans les contacts internes et externes.
