Assistant·e social·e / Case Manager (H/F/D)

CDD de 2 ans, prolongeable

50% évolutif

Vos missions

Assurer le suivi individuel (case management) des personnes accompagnées par la fondation, de l'évaluation initiale au bilan de fin de suivi

Soutenir le directeur dans la coordination des dossiers sociaux et administratifs

Assurer la liaison avec les partenaires externes (services sociaux communaux et cantonaux, offices AI, caisses de compensation, assureurs, réseau médical et thérapeutique)

Orienter les bénéficiaires dans leurs démarches auprès des assurances/institutions sociales suisses (AI, LAVI, AVS, assurance\-chômage, aide sociale, prestations complémentaires, LAMal, etc.)

Rédiger les rapports de situation, bilans sociaux et documents nécessaires au suivi des dossiers

Contribuer au développement des outils et processus internes

Représenter la fondation lors de séances de réseau, sur demande du directeur

Votre profil

Titre HES en travail social (assistant·e social·e) ou formation jugée équivalente

Une formation complémentaire en case management de santé/insertion est un atout

CAS LAVI un atout

Expérience confirmée en accompagnement social individuel, idéalement en lien avec les domaines de la santé, l’insertion et la précarité

Expérience de collaboration étroite avec une direction

Très bonne connaissance du système des assurances sociales suisses et des institutions sociales

Maîtrise de la constitution de dossiers auprès des assurances sociales

Aisance relationnelle avec un public en situation de vulnérabilité

Français courant indispensable

Bonnes connaissances de l'italien et de l'allemand – important
