Mission

En lien avec l’équipe pluridisciplinaire et sous la supervision des référent\-e\-s de situation, vous contribuerez à la prise en charge globale des bénéficiaires par :

  • La réalisation de soins de base et d’aide au ménage, dans une approche bienveillante et centrée sur la personne
  • soutien dans les activités de la vie quotidienne pour favoriser l’autonomie et le bien\-être
  • La transmission des observations pertinentes à l’équipe afin d’assurer une continuité et une cohérence des soins
  • La participation aux colloques interdisciplinaires et aux échanges de pratiques, pour contribuer à l’amélioration de la qualité de la prise en charge

Cette fonction valorise l’esprit d’équipe, l’écoute et la coopération interprofessionnelle, dans un environnement où chaque contribution est essentielle au bien\-être des bénéficiaires.

Profil

  • Vous êtes au bénéfice d’une attestation 120h auxiliaire santé de la Croix\- , Aide\-soignante, Aide\-familiale (adaptation du salaire et du titre en fonction du diplôme).
  • Expérience de 2 ans minimum souhaitée. Dans les soins à domicile : un atout.
  • sens de la relation, communication et négociation.
  • Proactivité, réactivité et orientation solution.
  • Polyvalence, flexibilité et adaptabilité.
  • Vous êtes dynamique, autonome et appréciez travailler en équipe.
  • À l’aise avec les outils informatiques.
  • Permis de conduire et voiture indispensables.
  • Pour les diplômes étrangers, une reconnaissance Croix\- est exigée ou être en cours de démarche.
