Während die Senevita Standorte für selbstständiges betreutes Wohnen und umfassende Pflege stehen, ist Senevita Casa die schweizweit führende Anbieterin privater Spitex\-Leistungen. Für die Senevita Casa Zug in der Region Ägerital und Menzingen suchen wir per sofort oder nach Vereinbarung eine/n Dipl. Pflegefachfrau/\-mann mit Fallführung 50\-100% Dipl. Pflegefachfrau/\-mann mit Fallführung 50\-100% Deine Aufgaben Bei unseren Kunden übernimmst du Einsätze in der Grund\- und Behandlungspflege Du spielst eine massgebliche Rolle dabei, sicherzustellen, dass unsere Kundinnen und Kunden zu Hause eine erstklassige und individuelle Pflege geniessen Du beratest und begleitest unsere Kunden und deren Angehörigen, anhand ihrer individuellen Bedürfnissen Du führst Bedarfsabklärungen mit RAI\-HC beim Kunden durch Du wirkst präventiven, diagnostischen und therapeutischen Massnahmen mit Förderung der Selbstständigkeit und Lebensqualität der Kunden im häuslichen Umfeld Du führst umfassende Pflegeanamnesen und Assessments durch Du leitest die unterstellten Mitarbeitenden in Bezug auf pflegerische Massnahmen an Du übernimmst administrative Aufgaben im Rahmen der Fallführung Das bringst du mit Ausbildung zur Dipl. Pflegefachperson Beratungskompetenz und Fähigkeit zur Anleitung von Klienten und Mitarbeitenden Mehrjährige Berufserfahrung in Langzeitpflege bzw.
