Pflegefachperson. Sie möchten Ihre Fachkompetenz in der Langzeitpflege einbringen, Verantwortung übernehmen und gleichzeitig aktiv zu einer hohen Pflegequalität beitragen? Dann erwartet Sie eine vielseitige Position mit Gestaltungsspielraum und fachlicher Verantwortung. Anforderungsprofil ##

  • Sie verfügen über eine abgeschlossene Ausbildung als Dipl. Pflegefachfrau / Pflegefachmann HF
  • Idealerweise bringen Sie bereits Berufserfahrung in der Langzeitpflege mit.
  • Sie übernehmen gerne Verantwortung und behalten auch in anspruchsvollen Situationen den Überblick.
  • Sie arbeiten selbstständig, strukturiert und lösungsorientiert.
  • Sie schätzen die Zusammenarbeit in einem interdisziplinären Team und bringen sich aktiv ein.
  • Sie begegnen älteren Menschen mit Wertschätzung und Einfühlungsvermögen.
  • Sie verfügen über gute Deutschkenntnisse in Wort und Schrift.

Aufgaben ##

  • Sie übernehmen die ganzheitliche und individuelle Pflege und Betreuung der Bewohnenden.
  • Sie steuern und begleiten den Pflegeprozess der Ihnen zugeteilten Bewohnenden und stellen eine professionelle Pflegedokumentation sicher.
  • Sie übernehmen Verantwortung als Bezugsperson und sorgen für eine kontinuierliche Betreuung.
  • Sie planen und begleiten Arztvisiten und stellen einen zuverlässigen Informationsfluss innerhalb des interdisziplinären Teams sicher.
  • Sie übernehmen die Tagesverantwortung und koordinieren die Aufgaben innerhalb des Tagesteams.
  • Sie wirken bei der Einschätzung des individuellen Pflegebedarfs mit.
  • Sie übernehmen Verantwortung für ausgewählte Fachthemen und bringen Ihr Fachwissen aktiv ins Team ein.
  • Sie führen Fallbesprechungen und Pflegerapporte durch und unterstützen damit eine professionelle Zusammenarbeit und Pflegequalität.
