Unser Auftraggeber ist eine moderne und innovative Spitex-Organisation mit rund 125 engagierten Mitarbeitenden. Im Mittelpunkt stehen die professionelle, individuelle und ganzheitliche Betreuung und Pflege von Menschen in ihrem vertrauten Zuhause. Dabei legt die Spitex grossen Wert auf eine hohe Pflegequalität, einen respektvollen Umgang sowie eine wertschätzende Zusammenarbeit im Team und mit verschiedenen Fachpersonen des Gesundheitswesens. Deine Aufgaben

  • Du stellst eine professionelle, individuelle und ganzheitliche Pflege und Betreuung unserer Klientinnen und Klienten in ihrem eigenen Zuhause sicher.
  • Du betreust Menschen mit unterschiedlichen Krankheitsbildern und bringst dein Fachwissen insbesondere in den Bereichen Somatik, Wundversorgung, Palliative Care und Demenz ein.
  • Du übernimmst eigenständig die Fallführung und koordinierst den gesamten Pflegeprozess verantwortungsvoll und zielgerichtet.
  • Du führst Bedarfsabklärungen durch, planst die erforderlichen Pflegemassnahmen und dokumentierst den Pflegeprozess sorgfältig im InterRAI Home Care.
  • Du erkennst Veränderungen im Gesundheitszustand frühzeitig und leitest entsprechende Massnahmen ein.
  • Du arbeitest eng und professionell mit Pflegeexpertinnen und -experten , Ärztinnen und Ärzten, Spitälern sowie weiteren Fachpersonen und Institutionen zusammen.

Dein Profil

  • Abgeschlossene Ausbildung als dipl. Pflegefachperson HF oder FH
  • Selbstständige, kompetente und zuverlässige Persönlichkeit mit ausgeprägtem Verantwortungsbewusstsein
  • Gute Anwenderkenntnisse im Bereich EDV
  • Führerausweis der Kategorie B
  • Erfahrung im Spitex-Bereich
