Deine Aufgaben --------------

  • Du bist für die Pflege und Betreuung unserer komplexen Fälle zuständig
  • Du trägst die Verantwortung für die pflegerische Diagnostik
  • Bei unseren Kunden übernimmst du Einsätze in der Grund\- und Behandlungspflege
  • Du beratest und begleitest unsere Kunden und deren Angehörigen
  • Du führst Bedarfsabklärungen mit RAI\-HC beim Kunden durch
  • Du erstellst Pflegedokumentationen und die individuelle Pflegeplanung
  • Du unterstützt die leitenden Personen beim Ausbau unseres internen Spitexangebots

Das bringst du mit ------------------

  • Ausbildung zur Dipl. Pflegefachperson
  • Mehrjährige Berufserfahrung in der Langzeitpflege bzw. Spitexbereich von Vorteil
  • Du kannst im RAI\-HC\-Bereich bereits Weiterbildungsabschlüsse vorweisen oder bist bereit, dich zeitnah weiterzubilden
  • Du bist belastbar, flexibel und selbstständiges Arbeiten gewohnt
