Dipl. Pflegefachperson HF ab 40% Aufgaben Fachperson Gesundheit: Fachverantwortung für die Pflegeplanung in der Pflegedokumentation und deren praktische Umsetzung und Überprüfung. Aktuell halten der Einstufung im BESA / RAI und die korrekte elektronische Erfassung Hauptverantwortung für eine lückenlose Pflegedokumentation Verantwortlich für professionelle Pflegeanleitung inklusive stetiger Weiterentwicklung Anführen von pflegerischen und medizinaltechnische Verrichtungen wie Medikamentenmanagement, Wundmanagement und Behandlungspflege Übernahme der Tagesverantwortung inklusive Führung des Pflegepersonals Interdisziplinäre Zusammenarbeit mit allen an der Behandlung und Pflege mitbeteiligten Personen und Institutionen, koordiniert die Kommunikation (Physiotherapie, Alltagsgestaltung, Küche, Technischer Dienst etc.) Gewährleistet eine der Situation angepasste Pflege und Betreuung gemäss Pflegeleitbildes, Bezugsplanung sowie neuster Standards Führt Fachberatungen für Bewohnende und Angehörige durch Überwacht den Gesundheitszustand der Bewohnenden Mitverantwortlich für das Erstellen, Aktualisieren, Umsetzung von Handlungsanweisungen sowie Pflegestandards Mitverantwortlich für die theoretische und praktische Förderung der Mitarbeitenden Aktive Teilnahme an Sitzungen und internen Weiterbildungen Mitverantwortung für Sicherheit und Gesundheit am Arbeitsplatz Anforderungen Abschluss als Dipl.
