Anforderungsprofil ##

  • Diplom als Pflegefachperson HF
  • Fundierte Berufserfahrung in der Langzeit- oder Spitexpflege
  • Weiterbildung im Bereich Demenz oder Bereitschaft zur entsprechenden Qualifizierung
  • Selbstständige, verantwortungsbewusste Arbeitsweise
  • Ausgeprägte Sozial- und Fachkompetenz sowie hohe Empathie
  • Führerausweis Kat. B und Bereitschaft, mobil unterwegs zu sein

Aufgaben ##

  • Ressourcengerechte Pflege und Betreuung von Menschen mit Demenzerkrankungen im gewohnten häuslichen Umfeld
  • Planung, Steuerung und Evaluation des Pflegeprozesses im Rahmen der Fallverantwortung
  • Durchführung von Assessments und Standortgesprächen (RAI-HC)
  • Unterstützung der Klient:innen im Alltag zur Förderung von Selbständigkeit und Lebensqualität
  • Enge Zusammenarbeit mit Angehörigen, Ärzt:innen sowie weiteren Fachstellen
  • Tagdienst von Montag bis Freitag (inkl. Feiertage)
