2.1 Pflege und Betreuung • Durchführung von Grund- und Behandlungspflege gemäss ärztlichen Verordnungen und Pflegeplanung • Professionelle Einschätzung des Pflegebedarfs und Erstellen individueller Pflegepläne • Unterstützung der Klient:innen bei Aktivitäten des täglichen Lebens (ATL) • Beobachtung, Evaluation und Dokumentation der gesundheitlichen Situation • Förderung der Selbstständigkeit und Ressourcen der Klient:innen • Anleitung, Beratung und Unterstützung von Angehörigen 2.2 Interprofessionelle Zusammenarbeit • Enge Zusammenarbeit mit Ärzten, Therapeut:innen und weiteren Fachpersonen • Teilnahme an Fallbesprechungen und internen Austauschsitzungen • Koordination notwendiger Massnahmen im Pflegeprozess 2.3 Dokumentation & Qualitätssicherung • Elektronische Pflegedokumentation gemäss internen Vorgaben • Einhaltung und Weiterentwicklung der Qualitätsstandards • Mitwirkung in Qualitätszirkeln, Teamentwicklung und Weiterbildungen 2.4 Organisation & Selbstständigkeit • Selbstständige Tourenplanung im Rahmen der Einsatzplanung • Sicherstellung einer effizienten und sicheren Arbeitsorganisation • Verantwortungsbewusster Umgang mit Material, Medikamenten und Hilfsmitteln

3. Anforderungen Fachliche Anforderungen • Diplom als Pflegefachperson HF oder Bachelor FH • Berufserfahrung in der Langzeit-, Akut- oder Spitexpflege von Vorteil • Kenntnisse in somatischer Pflege, Wundversorgung, Medikamentenmanagement • Gute Beobachtungsgabe und klinische Einschätzungskompetenz Persönliche Kompetenzen • Hohe Selbstständigkeit und Verantwortungsbewusstsein • Zuverlässige, empathische und belastbare Persönlichkeit • Kommunikations- und Teamfähigkeit • Flexibilität und Freude an einer abwechslungsreichen Tätigkeit Weitere Anforderungen • Führerschein Kat. B (obligatorisch für Spitex) • Gute Deutschkenntnisse in Wort und Schrift • Bereitschaft zu Fort- und Weiterbildungen
