Die öffentliche Spitex Wald Fischenthal ist für die ambulante Gesundheitsversorgung der Einwohnerinnen von Wald und Fischenthal verantwortlich. Die Spitexmitarbeiterinnen pflegen ihre Klientinnen und Klienten im eigenen Zuhause kompetent und einfühlsam, betreuen sie psychiatrisch oder unterstützen sie im Alltag. Die Spitex Wald Fischenthal ist Teil der gemeinnützigen Stiftung Drei Tannen, welche nebst der Spitex das neu gebaute Pflegezentrum Rosenthal führt und Alterswohnungen mit bedürfnisgerechtem Service anbietet. Dipl. Pflegefachperson HF/FH Somatik Deine Aufgaben: Individuelle Pflege & Betreuung: Du begleitest unsere Klienten mit Herz und Fachwissen in ihrem Alltag. Fallführung & Verantwortung: Du steuerst den Pflege- und Betreuungsprozess und übernimmst die Fallführung. Medizintechnische Massnahmen: Du setzt deine Expertise gezielt ein, um bestmögliche Versorgung sicherzustellen. Flexibilität im Einsatz: Abend- und Wochenendeinsätze gehören zu deinem abwechslungsreichen Arbeitsalltag. Interdisziplinäre Zusammenarbeit: Du arbeitest eng mit Kolleginnen innerhalb der Stiftung Drei Tannen, externen Partnerinnen und Angehörigen zusammen, um eine ganzheitliche Betreuung zu gewährleisten. Innovation: Veränderung verstehst du als Teil des Lebens. Du begegnest neuen Anforderungen offen, denkst innovativ und gestaltest Weiterentwicklungen aktiv mit. Was Du mitbringst: Diplom als Pflegefachperson HF/FH Erfahrung mit RAI HC und digitalen Arbeitsinstrumenten Eine kommunikative, teamfähige Persönlichkeit mit guten Deutschkenntnissen Führerausweis Kategorie B Eigeninitiative, Selbstverantwortung und Flexibilität - du packst an und findest Lösungen Und natürlich: eine gute Portion Humor - weil unser Job erfüllend, aber auch herausfordernd ist!
