zusammen mit Herz - dynamisch unterwegs Die Spitex Rothenburg bietet für die rund 8-000 Einwohner der Gemeinde Rothenburg Dienstleistungen in den Bereichen somatische Pflege, ambulante psychiatrische Pflege und Betreuung sowie Hauswirtschaft an. Die Pflegeeinsätze im Haus & Hof Hermolingen mit ca. 36 Bewohner und die Erbringung von Psychiatrieleistungen im erweiterten Umfeld runden unseren Spitexbetrieb ab und machen ihn zusätzlich zu etwas Besonderem.  Per 1.1.2027 bündeln wir in der Region Sempachersee unsere Kräfte und starten gemeinsam als neue, moderne und zukunftsorientierte Organisation Spitex Sempachersee Plus AG in die Zukunft.  Zur Verstärkung unseres Psychiatrie-Teams am Stützpunkt Rothenburg suchen wir          per sofort oder nach Vereinbarung eine engagierte  Dipl. Pflegefachperson HF Psychiatrie 40 - 90% Ihre Aufgaben Fachgerechte und ganzheitliche Pflege und Betreuung von Menschen mit einer psychischen Erkrankung - in ihrem gewohnten Daheim Übernahme der Fallverantwortung und Steuerung des gesamten Pflegeprozesses, inklusive Bedarfsabklärungen nach InterRAI-CMH Aktive Zusammenarbeit mit allen im Pflegeprozess Beteiligten Bei Bedarf Beraten der Mitarbeitenden aller Berufsgruppen aus dem somatischen Team  Im Psychiatrie-Team selbständige Planung und Koordination der verschiedenen Aufgaben und Klienten-Einsätze  Mitgestalten der Weiterentwicklung des dynamischen Fachbereichs Psychiatrie Sie bringen mit Diplom als Pflegefachperson HF / FH Psychiatrie Mindestens 2 Jahre Berufserfahrung im Fachbereich Psychiatrie zur Erfüllung der gesetzlichen Vorschriften, um Bedarfsabklärungen selbständig machen zu können  Erfahrung / Interesse im Kinder- und Jugendpsychiatrischen Bereich wünschenswert Hohe Belastbarkeit und grosses Verantwortungsgefühl Selbständige, strukturierte und vorausschauende Arbeitsweise Lösungs- und ressourcenorientierte Grundhaltung Teamfähigkeit Privatauto oder Roller Durch Ihre offene, fröhliche Art und eine klare Kommunikation finden Sie leichten Zugang zum Spitex-Team und allen weiteren Beteiligten Wir bieten Ihnen Arbeitstage Montag - Freitag, Fixtage möglich Sorgfältige Einarbeitung in das Aufgabengebiet  Motiviertes und hilfsbereites Team  Regelmässige Fachberatung / Supervision durch externe Person und Teilnahme am regionalen Netzwerk Psychiatrie Fachliche und persönliche Entwicklungs- und Weiterbildungsmöglichkeiten Wertschätzende Betriebskultur 5 Wochen Ferien, ab dem 50.
