Dipl. Pflegefachperson im Stunden oder Monats\-lohn 40\-100% \- Region Schwyz, Uri \& Glarus

Das bringst du mit

  • Du spielst eine massgebliche Rolle dabei, sicherzustellen, dass unsere Kundinnen und Kunden zu Hause eine professionelle und individuelle Pflege erhalten
  • Du führst Bedarfsabklärungen durch und erstellst eine individuelle Pflegeplanung
  • Du koordinierst die notwendigen pflegerischen Massnahmen und stellst die Zusammenarbeit mit Angehörigen, Ärztinnen und Ärzten sowie weiteren involvierten Fachstellen sicher.
  • Du führst pflegerische und medizinaltechnische Massnahmen fachgerecht und selbstständig durch
  • Als fallführende Mitarbeiterin steuerst du den Pflegeprozess deiner zugeteilten Kundinnen und Kunden, leitest situationsgerechte Interventionen ein, stellst eine fachgerechte und vollständige Dokumentation sicher und beteiligst dich aktiv an der interprofessionellen Zusammenarbeit.
  • Du bist im engen Austausch mit der Pflegedienstleitung und wirkst aktiv bei Massnahmen zur Qualitätssicherung und Qualitätsentwicklung mit

Das bringst du mit

  • Du verfügst über ein Diplom als Pfegefachfrau / Pflegefachmann HF oder eine gleichwertige, in der Schweiz anerkannte Ausbildung
  • Du verfügst über einen InterRAI\-HC Kurs oder bist bereit, diesen zu absolvieren
  • Du arbeitest gerne selbstständig, bist flexibel und organisierst deinen Arbeitsalltag zuverlässig
  • Du verfügst über gute mündliche und schriftliche Deutschkenntnisse und kannst dich sicher mit Kundinnen und Kunden, Angehörigen und Fachpersonen verständigen
  • Du zeichnest dich durch Empathie, Verantwortungsbewusstsein und Belastbarkeit aus
