Pflegefachperson Spitex mit Fallführung (70–100%) Seewadel \- Zentrum für Gesundheit und Alter ist eine öffentlich\-rechtliche Institution der Stadt Affoltern am Albis im Bereich der ambulanten und stationären Pflege und Betreuung. Menschen dabei zu unterstützen, möglichst lange selbstbestimmt und sicher in ihrem vertrauten Zuhause leben zu können, ist für uns eine Herzensangelegenheit. Mit unseren ambulanten Pflege\- und Betreuungsleistungen begleiten wir Klientinnen und Klienten bedarfsgerecht und personenzentriert im Alltag. In dieser verantwortungsvollen Funktion übernehmen Sie die eigenverantwortliche Fallführung und gewährleisten eine bedarfsgerechte, qualitativ hochwertige Pflege und Betreuung im häuslichen Umfeld. Zur Verstärkung unseres Somatik\-Teams in der Spitex Seewadel suchen wir per sofort oder nach Vereinbarung eine engagierte Persönlichkeit als: Dipl. Pflegefachperson Spitex mit Fallführung (70–100%) Ihre Kernaufgaben • Verantwortung für die Fallführung inklusive Durchführung von Abklärungs\- und Beratungsgesprächen mittels InterRAI\-HC\-Abklärungen • Planung, Umsetzung und Evaluation der Pflegeprozesse • Sicherstellung der bedarfsgerechten und klientenorientierten Pflege im häuslichen Umfeld sowie Anpassungen an veränderte Situationen • Interprofessionelle Zusammenarbeit mit Hausärzten, Angehörigen, Institutionen und weiteren Fachpersonen • Aktive Mitwirkung an der Qualitätssicherung und Weiterentwicklung des Fachbereichs Somatik Ihre Kompetenzen • Abgeschlossene Ausbildung als Pflegefachperson HF/FH sowie Erfahrung in der Fallführung und Pflegeprozesssteuerung (RAI\-HC) • Fundierte Fachkenntnisse in der Pflege und Betreuung von Menschen in komplexen Situationen im ambulanten Setting • Hohe Sozialkompetenz mit empathischem und wertschätzendem Umgang, professioneller Beziehungsgestaltung sowie personenzentrierter Beratung und Begleitung • Sehr gute Deutschkenntnisse in Wort und Schrift sowie versierte PC\-Anwenderkenntnisse • Ausgeprägte Kommunikations\- und Teamfähigkeit sowie ein hohes Mass an Selbstständigkeit, Verantwortungsbewusstsein und Belastbarkeit Ihr Vorteil • Eine anspruchsvolle Aufgabe in einem motivierten Team einer dynamischen Organisation • Ein positives Arbeitsklima mit attraktiven Anstellungsbedingungen und Sozialleistungen sowie einer überdurchnittlichen Entlöhnung • Interne und externe Weiterbildungsmöglichkeiten, Supervision • Ein Arbeitsplatz in Bahnhofsnähe mit guter Anbindung an öffentliche Verkehrsmittel Sind Sie interessiert, sich in einem zukunftsgerichteten Betrieb einzugeben?
