Menschen mit Demenz brauchen Menschen, die für sie da sind Die Sonnweid gilt als eine der führenden Institutionen für Menschen mit Demenz. In unserem Kompetenzzentrum betreuen und pflegen unsere 330 Mitarbeitenden aus verschiedenen Fachbereichen 174 Bewohnerinnen und Bewohner. Hausärzte und Alterspsychiater stellen die medizinische Versorgung sicher. Das differenzierte Wohn\- und Betreuungskonzept ist dem Stadium der Erkrankung angepasst. Unsere Semiakutstation bietet ein vorübergehendes Zuhause für Menschen mit besonders herausforderndem Verhalten, während die Tag\-/Nacht\-Station betreuende Angehörige mit Tages\- und Ferienaufenthalten entlastet.

Unser Leitbild, das die Bedürfnisse des an Demenz erkrankten Menschen in den Mittelpunkt stellt, bildet die Grundlage für unser tägliches Handeln. Mit Freude und Überzeugung setzen wir alles daran, dass sich die Bewohnenden wohl und in der Sonnweid zu Hause fühlen. Dabei respektieren wir uns gegenseitig in unserer Individualität und unterstützen uns in herausfordernden Situationen.

Ihre Aufgaben

  • Sicherstellen einer individuellen und beziehungsorientierten Betreuung, Pflege und Begleitung unserer Bewohnenden während der Nacht
  • Fachliche und personelle Führung unseres Nachtwachen\-Teams mit 15 Mitarbeitenden
  • Mitarbeit in der Betreuung und Pflege
  • Interdisziplinäre Zusammenarbeit im Team und mit den Ärzten und weiteren Partnern
  • Sicherstellung der Pflegedokumentation und eines guten Informationsflusses

Sie bringen mit

  • Abgeschlossene Ausbildung als Pflegefachfrau/\-mann HF, DN I/II, AKP, PSKP, KWS, IKP
  • Offener, klarer, wertschätzender und partizipativer Führungsstil
  • Freude an beziehungsorientierter Arbeit
  • Offenheit und Kreativität im Umgang mit den Bedürfnissen von Menschen mit Demenz
  • Selbständige und verantwortungsbewusste Arbeitsweise
